I killed the merchant in Resi 4 by mistake once using my knife. It was on Pro mode as well so it was even worse

Yeah, I've had to reload my save a few times during a Professional Playthrough of Wii edition a few times because I would quickly raise my Wiimote as I left the Buy Screen and end up slashing the Merchant in the damn throat.

In CVX, leaving the grenade launcher on Rockfort island and not being able to defeat the tyrant on the plane. I had to start the whole game again.

Yeah, one little mistake can mess up whole game D:
