Professional difficulty
To unlock the hardest difficulty in the game, Professional difficulty, you'll need to have successfully complete the game once on any difficulty setting. Upon doing so, Professional difficulty will be an available option when starting up a New Game.

'Infinite Rocket Launcher'
You might as well kiss goodbye to any semblance of challenge from the game. Having said that, it does feel mightily rewarding to wipe that smug face of Salazar with a few well-placed missiles to the gob. You'll unlock the 'Infinite Rocket Launcher' after completing the game for the first time. However, you'll need to scrape together a very daunting 1,000,000 PTAS if you want to ever lay your hands on it.

'Separate Ways' campaign
In order to unlock this bonus campaign featuring Ada Wong as the lead, playable protagonist, you'll need to have completed the game once on any difficulty setting. It'll be available for selection on the main menu.

Note: The GameCube version of Resident Evil 4 does not include this mode.

'The Mercenaries' mode
After completing the game once on any difficulty setting, select it from the main menu. Earning a four-star rank on certain stages will unlock additional characters, such as Ada Wong, Albert Wesker, Hunk and Jack Krauser.

Bonus outfits for Leon and Ashley
Both Leon's RCPD outfit and Ashley's 'pop star' are unlocked after completing the game for the first time on any difficulty setting. They can be selected when initiating a New Game+. However, once they've been selected, you cannot change the outfits again until either your current playthrough has been finished or you begin a fresh one.

'Mathilda' handgun
Rumoured to be named after Natalie Portman's character from Luc Besson's Léon, the Mathilda pistol is unlocked after completing the game on any difficulty setting, and can be acquiring after continuing a New Game+ and visiting the Merchant.

P.R.L. 412
The The Plaga Removal Laser 412 is a monster of a gun. You'll earn this ferociously powerful weapon after successfully completing the game on the Professional difficulty setting and speaking with the Merchant.

'Handcannon'
Potentially a tricky one, this. You'll need to first unlock the Mercenaries mode by completing the game on any difficulty setting, then you'll be required to earn — brace yourself — a five-star rank on all four stages for each of the five playable characters. Upon doing so, you'll be able to purchase the Handcannon from the Merchant during the campaign mode for precisely zero PTAS.

Chicago Typewriter ('Tommy Gun')
Successfully complete Ada's Separate Ways mini-campaign, and you'll be able to purchase the Chicago Typewriter from the Merchant for the obscenely expensive price of 1,000,000 PTAS.

'Punisher' handgun
Find and destroy ten out of fifteen of the Blue Medallions scattered throughout the farm and cemetery areas of the first chapter, and the Merchant will let you have this relatively weak handgun for free. Finding fifteen out of fifteen of the Blue Medallions and speaking with the Merchant will mean that he will provide you with an upgraded version of the gun. Follow this guide to track down each of the Blue Medallions.
